The Pomeranian Model of Integrated Care project, implemented by the Department of Allergology and Pneumology of the University Clinical Center of the Medical University of Gdańsk, won the main prize and a distinction in the cost- effectiveness category in the Polish part of the prestigious international competition Value in medicine: Dragon’s Grant & Endorsement.
The competition is addressed to institutions health care implementing practical solutions within the Value Based Healthcare (VBHC).
The Pomeranian model of integrated care for patients with advanced COPD was introduced in order to reduce the number of exacerbations and costs of care, and to improve the quality of life of patients. The project is conducted by the Clinic of Allergology and Pneumology with the participation of the Polish Society for Health Programs, in cooperation with the Center for Competence Development, Integrated Care and e-Health.
