The Pomeranian model of integrated care for patients with advanced Chronic Obstructive Lung Disease (COPD) won the European prestigious international competition Value Based Healthcare Dragon’s Grant & Endorsement. The form of the competition was similar to a television program, in which entrepreneurs had to arouse the interest of potential investors in just a few minutes.
The Pomeranian model of integrated care for patients with advanced COPD was introduced in 2010-2012 in order to reduce the number of exacerbations and costs of care and improve the quality of life of patients, informs the university.
The project is carried out by the Clinic of Allergology and Pneumology of the University Clinical Center in Gdańsk – a clinical hospital at the Medical University of Gdańsk. It is also attended by the Polish Society for Health Programs and the Center for Competence Development, Integrated Care and e-Health.
